I would like to develop a form for our surgeons to take to the donor hospital to document that ABO verification has taken place. Is anybody already using a form they would be willing to share? Where do you keep your documentation once the surgeon has filled out the form?&lt;P&gt;How are other centers meeting the CMS requirement that on-site ABO verification be documented? Is your process the same for all organs?
