Does anyone have any experience using a living liver donor who is hepatitis B core antibody positive when the recipient is antibody negative? With deceased donor liver transplants we use such a donor only when the recipient is antibody protected or in the event of a fulminant who will die without a graft. Please share any protocols you have re: the heptaitis B core antibody positive donor into the hepaitis B surface antibody negative recipient - HBIg w/ 2 oral agents? HBIg IV or IM? And if you have used these positive donors with negative recipients, has insurance covered the expense of treating the recipient to prevent donor transmission of HBV?

Denise,

Do you assess for both IgM and IgG in the living donor? I would think that all things considered, an old exposure to Hepatitis shouldn't be a significant problem.
