We do a yearly plan for hospitals, developed jointly with them.  I know in our larger hospitals, the plan is revisited at donor councils and in the course of other meetings, education delivery, etc.

How often is a plan re-assessed in most hospitals - looking at all categories?  Does anyone have a great tool they use for this?
