It is easier with a smaller center....however, we (the clinical coordinators) meet with our financial coordinator every two weeks to go over all the patients (about 300).  This helps us both to make sure patient statuses are where they should be and serves as a double check for the clinical coordinators to follow up on patients who are on medical hold, workup, etc.  We also have excellent relationships with our dialysis units and their social workers who keep us updated on the patients regularly.

I think every program needs to find what works best for them.  we started with a small WL management subgroup & looked at our list, where the top issues were, and also looked at the CMS requirements. Our director wrote policies regarding wait list management & then our Clinical Council wrote additional "guidelines as well for Wait List Management.  the pre-coordinators all have their census of people in the evaluation process as well as some on the wait list that they are 'supposed' to manage.  the difficulty is assuring that they keep these patients on their radar to assure they are healthy enough for transplant.   part of my job is to be the gate keeper for the list & assure we are following our policies.
