We are trying to streamline our various organ transplant departments to use the same consent form. Interested to know how other hospitals with multiorgan programs are dealing with this issue. I would welcome any suggestions from multiorgan centers.

Maureen, our center is also in the process of doing this. Our heart program already uses a form. We in the intraabdominal program will most likely modify this form so we're all using a similar document. I will email you the document.

Our center does liver/intestine/kidney/pancreas and heart transplant. We use the same surgical consent form. However, the Evaluation Consent form required by CMS cannot be the same due to the differences in selection criteria, risk factors, SRTR data, etc.
