I am interested in hearing from centers regarding ABO verification in the field. I would like to develop a form for our surgeons to take to the donor hospital to document that ABO verification has taken place. Is anybody already using a form they would be willing to share? Where do you keep your documentation once the surgeon has filled out the form?

How are other centers meeting the CMS requirement that on-site ABO verification be documented? Is your process the same for all organs? I am a liver transplant coordinator but would like to use something for all abdominal organs.
