once patients are accepted in your center as an appropriate candidate for the deceased list -who & how is the summary individualized?  I have heard that some centers have a "listing sheet" that the surgeon fills out & signs and then the coordinator enters the info into UNET.

I would be interested in see everyones process.

At our center we fill out after a patient has been accepted, we fill out listing paperwork and have our medical director sign it. It is then faxed to our OPO and the listing process occurs there. I have heard recently that this process will be changing soon and that the transplant coordinators will be responsible for listing potential candidates now...
